Not established. The opening doha speaks in the first person, so the work has a human author, but no printed or digital source names a credible one. Names that circulate online are mutually contradictory and appear to be website metadata rather than attributions; one commercial listing gives a publisher as "author". The text appears anonymously in the compilation it is taken from here.
कालThe earliest printing we could document is the first edition of Randhir Prakashan’s "51 Chalisa Sangrah", Haridwar, 2000, read off the imprint page of the scan. The related Shyam Chaurasi, from which this chalisa borrows its opening doha almost word for word, first appears in print in 1989. Google Books holds nothing matching this text before 1985, and the Internet Archive holds nothing at all about Khatu Shyam before 1980: the whole printed tradition about this deity begins in the 1980s.
Modern Hindi (Khari Boli) with light archaising, not the Awadhi of the older chalisas
